This screening tool was developed by Canines 4 Comfort to help individuals and families learn if they might qualify for a Psychiatric Service Dog or an Emotional Support Animal through our organization. Answering these prescreening questions will help us determine whether we may be able to help you with the right animal for your personal needs. 

NOTE: Only by completing the application process, will our team have sufficient information to assess whether you qualify for the next interview step. Completing the application does not guarantee we will have a PSD or an ESA dog matching your needs. There is NO CHARGE to complete the application and interview processes.
